They are currently seeking a Full-time Family Nurse Practitioner for a mobile van traveling throughout Suffolk CountyRequirements:Van will be parked in Patchogue but will travel to different sites daily NP will assess, evaluate patientsGoal is to connect patient with a permanent health home at one of the clinic sites Rewarded experience connecting with underserved communities
