Looking for an Case Management Nurse interested in the Santa Clara area of New Mexico. This facility has 25 acute-care beds as well as 66 long-term care beds. With this facility, you get to know all of your coworkers and have a very strong family feel.

The Case Manager is responsible to assist in the development, planning, coordination and administration of the activities of Utilization Review and Discharge Planning.
