They are currently seeking a full-time Social Worker LCSW to join their team in Suffolk County!Essential Duties and Responsibilities: Interviews patients and family membersObtains psycho-social histories of patients Provides individual, group, family, and material counseling Acts as case manager to help patients negotiate the treatment system Responsible for keeping patient records current Participates in team meetings Develops treatment plans with patients as required Writes reports and documentation as required Consults with other staff regarding treatment of patients Coordinate care with other community agencies Minimum Education Requirement: Master's Degree in Social Work and LCSW requiredMinimum Work Related Experience: 2-4 years Preferred Work Experience: 5-8 years Licensure/Certification/Registration Requirement: Valid Social Work License in the State of New York
