As a Hospice Registered Nurse, you will: Give direct nursing care to hospice patients. Complete admission, discharge and nursing note documentation. Complete daily RN assessments on all patients. Supervise meal service and feeding of patients. Take report on patient at time of transfer/admission to the inpatient unit and at the beginning of each shift. Give report on patient at end of each shift. Forward requisitions for supplies to the assigned PCS or Inpatient Manager Forward all requests for contracted services to the Inpatient PCM or administrator on-call. Coordination and Communication with Interdisciplinary team members for Physician, Social Worker, Spiritual Care Coordinators, Bereavement Coordinators, Volunteer and Dietician services, as needed. Make rounds on all patients at the shift beginning and then periodically during each shift. Receive and carry out all Physician’s orders. Ensure the Hospice Plan of Care is implemented for all patients. Accompany physicians on rounds at the inpatient unit. Address and handle patient/family questions. Forward all complaints/grievances to the Inpatient Patient Care Manager or Administrator On-Call. Qualifications Current and unrestricted Registered Nurse licensure Bachelor’s degree and C.H.P.N. (Certified Hospice and Palliative Nurse) certification preferred Minimum of one year general nursing preferred with current Hospice, home health, medical, surgical or critical care experience Experience with pain and symptom management interventions Ability to work within an interdisciplinary setting Understanding of Hospice philosophy, principles of death/dying
