Job Category Physical Therapist Profession Home Health Specialty Shift 8 D Duration 12 Weeks Start Date 01/08/2018 Pay Rate $45.00 - $47.50 Job Description & Requirements Physical Therapist - Home Health - Travel (PT - HH) o Assesses patient's needs to ensure quality physical therapy service
o Plans and supervises care for designated patients in accordance to those standards set by this hospital and the (APTA)
o Implements a therapeutic program with individuals
o Evaluates effectiveness of plan of care; ensures and oversees that responsible care is rendered to patients.
o Self-Management/Professional Development - Holds self accountable for the delivery of responsible care; demonstrates effective communication skills and interpersonal relationships.
o Must be familiar with EMR documentation systems and be familiar with OASIS C2 assessments Required Qualifications *All Candidates MUST have a clean Driving Record and Valid Drivers' License in order to be considered*
2 years' minimum recent Home Health experience - Required
Prior travel experience - Required
BLS must be AHA
PT Michigan license - Required
