Job Category Physical Therapist Profession Skilled Specialty Shift 8 D Duration 13 Weeks Start Date ASAP Pay Rate $42.00 - $44.50 Job Description & Requirements Physical Therapist - Skilled - (PT - Skilled) -- Screening - Genesis Rehab Services (GRS) requires the following: All requirements listed on agency checklist must be uploaded into xxxxxxxxxxx* prior to start. All agencies are responsible for being familiar with any/all state specific requirements - including background check procedures - for their travelers for all assignments.- Overtime - All overtime (any time over 40 hours in a week) must be approved by a Program Manager in advance of Contractor working that overtime.- Travel Time - Travel time/mileage to/from home is not paid by GRS. Travel time or mileage between centers will be considered on a case by case basis, but must be approved prior to confirmation of the assignment. If there is a pre-approval of travel time/mileage, please include that in the notes section of the confirmation.- Coverage Requests - Contractors are expected to comply with coverage requests, and should be willing to provide service at facilities within a thirty-mile(30) radius of their original assignment. Please note that all mileage reimbursement paid by Genesis Rehabilitation Services will be paid at $.405 per mile and must be approved by the manager.- Orientation Time - For Traveler and Ongoing positions, Agency will provide eight (8) hours of orientation time, at no charge, for Contractors on their first assignment to GRS. For casual or per diem assignments, Agency will provide three (3) hours of orientation time, at no charge, for Contractors on their first assignment with GRS. GRS does expect that the Contractor is paid by the Agency for this time. Contractor may be asked to treat patients during this orientation time and they are expected to comply.- Cancellation - Traveler assignments require a 30 day notice. PRN assignments require a 24 hour notice. If the Agency cancels the assignment with less than a 24 hour notice, Agency will provide GRS with an invoice credit for the remaining time up to 8 hours.Holidays: Thanksgiving Day, and Christmas Day. GRS will pay all-inclusive + $10/hr for time worked on holidays or offer the therapist weekend hours to fulfill any weekly guarantee, if the therapist decides to not work additional hours offered, weekly guarantee is not valid.Time off requests: Genesis Rehab Services will not honor Time off Requests not included in the original submittal. Any time off requests made after the candidate has been confirmed must be presented to staffing coordinator for approval. If time is not approved and the candidate does not work, there will be a 6 hour invoice credit issued for each day missed.
