Description
We are recruiting for a Nurse Advocate to join the Continue the Connection Program within our Marketing and Communications Department in Louisville, KY. Summary: The Nurse Advocate is responsible for following up on patient satisfaction post agency discharge. This individual assures patient needs are met post discharges on a recurrent ongoing basis. Essential Functions: Establishes, maintains and develops relationships with discharged patients. Accurately and professionally completes phone calls to patients post discharge at predetermined intervals and completes Discharge Questionnaire. Develops strategies to improve patient satisfaction. Develops strategies to include proper follow-up with physicians, account managers, and agency Director of Operations regarding results of discharge questionnaire. Provides updates, coordinates, and communicates patient needs, concerns, and interests to Regional Administrator, Director of Operations and Area Directors of Sales. Obtain physician's orders for home health services and obtain a co-signature by an appropriately licensed professional. Assist in strategies to communicate regularly with patients who request follow-up services on the status of communications with physicians' offices. Communicates regarding patient admission status to the referral source and other appropriate parties. Complete and submit timely paperwork necessary for documentation and record keeping. Performs other related duties as assigned or requested. Knowledge/Skills/Abilities: Must have excellent interpersonal, communication organizational skills. Ability to handle multiple projects and deadlines. Proficient presentation, facilitation, and public speaking skills, organizational and motivational skills. Ability to respond promptly and effectively to patient questions and concerns. Computer competence with MS Office applications. Ability to use customer service principles and listening skills to effectively communicate via the telephone with customers across a large geographic coverage area. Self directed, flexible and able to work in fast paced environment with a fluctuating work load. Demonstrated ability to work independently and cooperatively as an effective team member. Ability to maintain confidentiality. Ability to be flexible in work hours and travel locally, as needed. Demonstrates commitment, professional growth and competency. Promotes company philosophy, mission and administrative policies to ensure quality of care. Must read, write and speak fluent English. #MON-CORP Qualifications: Associate's or Bachelor's degree. Registered Nurse with active professional licensure required. 5+ years post-acute healthcare experience. Background in case management or utilization review is preferred. Triage and telephonic call center background is preferred. First Shift Schedule Depending on a candidate's qualifications, this position may be filled at a different level.
