Rate Information:  Please submit AHSA Rate sheet with provider presentation
Start Date:  6/11/2018
Duration:  Start as soon as credentialed (6-8 wks) to cover coining until physician returns from FMLA

Shift:  
Shift Notes:  Shifts could be 7a-7p, 8a-7p, 7a-5p, & 7p-7a
On Call Requirements:  No Call
Weekend Requirements:  As Needed
MANDATED Job Requirements:  BC - Required (no BE)
ME License - Required
DEA - Required
ACLS - Required
Locum must be able to cover a 4 to 16 shifts per month to be consider - Required
Registration/Certification Requirements:  Board Certified
State License(s):  Maine
Unit Information:  •   Area of Maine known for its scenic cliffs, river, and all around outdoor appeal. They also have an abundance of restaurants and shops and a community theater
Job Description:  25 Bed Critical access hospital
EMR - EPIC
Coverage is for FMLA
Vent Management required
ICU has 4 ICU beds and 4 Tele beds
