Job Description
Performs daily round on new and existing patients
Completes discharge and death summaries
Activities include:
-Consultations as requested
-Assistance in case conferencing
-Clinical supervision
-Clinical over site
-Assist Medical Director with patient site visits including face to face visits

MANDATED Job Requirements
A minimum of 1 years experience in Hospice and or Palliative Care - Required
Knowledge of Medicare and Medicaid billing - Highly Preferred
EMR Experience - Highly Preferred
