This Geriatric NP need could be for locum tenens, locums to perm or permanent placement.

It is located on the coast of the Pacific Northwest. This beach side city is filled with small town charm. Unique dining options as well as great breweries and wineries within the area. Also sits within a short drive to Portland, Oregon and Vancouver, Washington!

*NP in this role works collaboratively with interdisciplinary team and primary care provider to provide care for geriatric patients. This position is a third palliative and the rest is geriatric.

*Four day work weeks! Schedule is Monday-Thursday OR Tuesday-Friday; 8a - 5p.

*NO WEEKENDS AND NO CALL!

*Majority of the work takes place within clinic setting with small percentage of home visits (5 to 10 per week).

*NP counsels with patients, families and caregivers regarding aging and care issues.

*Minimum of 1 year of experience working with medically frail, elderly patients. Palliative care experience preferred and/or interest in palliative medicine.

*EPIC EMR.

*Support staff is RN staff, social worker, home services, therapy and rehab staff, personal care aides. 6 patients per day.
