The group's focus is on outcomes, not on volume of patients.

Key Characteristics of a the Practice:
Physician run organization with peer collaboration
New patients are scheduled at 40 minute intervals and follow ups are 20 minutes
On average the daily schedule is less than 20 patients per day
Balanced work life; Clinic Hours 7:30 – 5 Monday – Friday
Practice Call – Phone call only 1:8 or less
Exclusive focus on complex senior patients
Small panel size is: 400-500 patients
