Nurse Practitioner Hospice Willamette Valley Oregon
Hospice team seeks nurse practitioner to support patients and their families through a holistic and collaborative end of life care model. The program operates a hospice house in a 12-bed in-patient facility serving patients in need of acute symptom management.
Join a team comprised of the Medical Director, Nurse practitioner and two physicians.
Responsibilities include:
Medical management of outpatient census in with hospice team
Daily rounds for symptom management
Working with Oregon Vital Events Registration System (OVERS) for death certificates
Visits in the field
Requirements:
BLS Certified
Oregon Nurse Practitioner License
Current certification as FNP or ANP
ACHPN certification preferred or ability to obtain in 1 year
Hospice experience preferred but willing to train an appropriate candidate
