This 80-bed skilled nursing facility in Northern California works hard to provide a continuum of care that bridges the gap between hospital and home. Skilled therapists and nurses partner with patients and their families to create tailored plans that incorporate individual preferences, abilities, and goals. In addition to short-term rehabilitation, staff provides long-term care services, outpatient rehabilitation, physical, occupational, and speech therapy as well as memory care.
