Physical Therapy Assistant needed for Home Health in White River Junction, VT
Day shift and rotating weekends
Must have COVID 19 vaccination
Position Objective:
Physical Therapist Assistant will maintain a case load of home care clients through assessment, plan of care, coordination and implementation, of evidence based care for each client, with the support of an interdisciplinary care team. This includes but is not limited to: appropriate treatment intervention, evaluation of outcomes, and utilization of services to ensure that patient care needs are met. Will participate in weekend rotation.
Performance Expectations:
· Clinical assessment and treatment of patients referred by a licensed physician.
· Evaluates patient's need for physical therapy and develops appropriate treatment plan. Refers patient to other professionals or agencies when appropriate.
· Provide appropriate care and treatment to patients based on physician care plan, agency policy and professional standards of care.
· Coordinate and communicate treatment activities with health care team members and the patient, instructs family members, PT's, Home Health Aides, and other caregivers in appropriate physical therapy techniques.
· Contribute to the professional development of new staff and health professional students.
· Weekend rotation expected.
Education and Licensure Required: Associate’s Degree in PTA
Must be licensed in NH
& VT, hold a Valid Driver’s License and agency level auto insurance
