- Location: Chiloquin, OR – 20 minutes NW of Klamath Falls, OR
- Duration: June – August (Option to extend) – searching for perm (open for locum to perm)  
- Reason for coverage: Two locum contracts are expiring June 3 – been there 1.5 years (providers family situation changed, need to be closer to home)
- Schedule: Monday – Friday 7a-4.30p (no weekends – no call)
- Call: N/A
- Patient volume: 20 patients per day
- Scope: Bread and butter outpatient primary care
- EMR: NextGen
- Hospital: Outpatient clinic
- Group/support: RN/FNP – 2 FNP, Medical Director – DNP, few specialized providers that rotate through
- Requirements (boards, license): Board Certified or Board Eligible, current DEA, license – out of state – OR license within 90 days – ACLS certification – fully vaccinated.
