Los Altos Sub-Acute and Rehabilitation Center located in Los Altos, California is dedicated to providing quality health to those in need. Our goal is to help individuals achieve their highest possible functioning level and to assist them in returning home whenever practical. Los Altos Sub-Acute & Rehabilitation Center provides the most comprehensive rehabilitation and nursing programs for Orthopedic, Neurological, Pulmonary, and other needs, as well as rehabilitation programs for those with complex medical needs. We offer comprehensive skilled nursing, physical rehabilitation and stroke rehabilitation programs.
