Physical Therapist Assistant needed for home health setting based in Randolph, VT!
Day shift and rotating weekends
16 week assignment
36 guaranteed hours
Mileage Reimbursed
Must be licensed in NH & VT
Service areas include: Randolph, Tunbridge, Strafford, Sharon, Pomfret, Royalton, Bethel, VT areas, and other towns may be included based on patient needs.
Physical Therapist Assistant will maintain a case load of home care clients through assessment, plan of care, coordination and implementation, of evidence based care for each client, with the support of an interdisciplinary care team. This includes but is not limited to: appropriate treatment intervention, evaluation of outcomes, and utilization of services to ensure that patient care needs are met. Will participate in weekend rotation.
