Required:

• Current State Medical License - South Carolina
  
• Board Certification/Board Eligible: BC or Board Eligible with experience
  
• DEA/CSR Requirements: Yes
  
• ATLS Certification
  
• DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
  
• Current Flu Vaccine
  
• Must be fully vaccinated for Covid 19
