- Coverage Needed:
- Schedule:
- 8:00 am - 5:00 pm
- Call as needed for Cath Lab / Office
- Exact Dates:
- October 23-27
- November 6-10
- November 20-24
- November 27-Dec 1
- Clinical Information:
- EMR= EPIC
- Support Staff: Mix of RNs and MAs
- 6 Physicians and 3 APPs
- Will work in Inpatient and Outpatient
- Special procedures: Interventional
- Required:
- Interventional Cardiology skills required &All typical procedures necessary
- Must be Board Certified
- Must have IN license to be considered and presented.
- CV must bein Month / Year format
- Must have license in hand at time of submission, or proof that the license application has been submitted
- Any gaps greater than 30 days will require an explanation on the CV
- DEA must remain in state to maintain privileges.Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
- Current Flu Vaccine
- Must be fully vaccinated for Covid 19
- Exemptions only allowed for religious or medical reasons, once approved by the facility.
