Description
A health services network in Connecticut is actively seeking a licensed Speech Language Pathologist (SLP) to join one of their Acute Rehab facilities in Fairfield County. In this role, the Speech Language Pathologist (SLP) will be responsible for planning, organizing, implementing, and documenting specific evaluation & treatment programs for patients of varied ages and disabilities.
About the Opportunity:
- Start Date: ASAP
- Assignment Length: 12 to 16 weeks
- Setting: Inpatient Acute Rehab / Acute Care
- Schedule: Monday to Friday
- Hours: 8am to 4pm
- Caseload: 6 to 10 patients
Responsibilities:
The Speech Language Pathologist (SLP) will:
- Obtain pertinent medical and social history
- Perform thorough and accurate evaluations using techniques according to current professional and health system standards to objectively measure physical and functional status
- Determine functional and measurable short and long term goals and designs an appropriate treatment program
- Perform treatment as outlined in the initial evaluation
- Modify treatment, as necessary; update goals and re-evaluate discharge plans throughout the course of treatment according to current professional and health system standards
- Perform accurate clinical documentation is timely, objective and complete
- Perform other duties, as needed
Qualifications:
- Graduate of an accredited curriculum with a Master's of Speech Pathology
- CT Speech Language Pathologist (SLP) license
- Certificate of Clinical Competency (CCC)
- BLS certification
- Microsoft Office proficient
- Solid assessment and documentation skills
