Position Details:

• Position Type: Full-time, recruiting for an Inpatient Palliative Care Advanced Practice Provider (APP).


• Scope of Care: Provide holistic care, addressing the physical, functional, psychological, and spiritual needs of individuals with serious illnesses.


• Patient-Focused Approach: Utilize a person-family-centered approach to care, focusing on symptom relief and alleviating the stress and burden associated with a disease process.


• Responsibilities:
  
  
  
  • Perform thorough history and physical examinations on patients.
  
  
  • Timely dictation of appropriate hospital reports.
  
  
  • Establish presumptive diagnoses, order necessary laboratory studies, and manage patient problems under physician supervision.
  
  
  • Initiate and coordinate laboratory, radiological, and special examinations or tests required for evaluation.
  
  
  • Provide counseling and instruction on patient problems, with referrals to other services as needed.
  
  
  • Offer follow-up care in accordance with the physician's plan of care.
  
  
  • Coordinate care with discharge planners for transitions to Nursing Homes, etc.
  
  
  
  

Qualifications:

  • Prior experience with end-of-life and chronic illness is preferred.
  
  
  • Demonstrated compassion for patients facing serious illnesses.
  
  
  • Strong communication skills to effectively interact with patients and their families.

  Licensure: Possess an Indiana Medical License.

  
  


• 
  

  Certifications: Maintain BLS/ACLS certifications.
