Description Location: 50 minutes from Portland, OR Duration: We likely will have a need for 6 – 9 months Reason for coverage: Loss of provider, increased census Schedule: Our Clinic is open for Provider schedules 830 – 7 M,T, TH, F, 830 – 530 on Wednesdays, and 830 – 2 on Saturday Shift: We are flexible within the above time frames. Call: No Patient volume: For a full day, around 17 Scope: All Ages or will consider Adult Only EMR: Next Gen Hospital: Referrals to Salem Health, Silverton (Legacy) or Stayton (Santiam) hospitals depending on where a patient lives. Group/support: We have a cadre of nurses, medical assistants, and other licensed medical providers on site Requirements (boards, license): Licensed to practice in Oregon as NP with DEA designation Skills Minimum of 2 years * Some information such as the email and phone number are hidden unless you are registered and logged-in
