• Practice Setting: Hospital-Based. Providers will read for all three locations but will be primarily based at the main hospital.

Submission Requirements:

• Board Certification: Required


• IL State License: Required


• BLS Certification: Required


• Conversion Rate Cap: $25,000

Additional Information:

• Estimated Credentialing Timeframe: 3 months


• Reimbursement: Flight, Hotel, Rental Car, and Mileage (billed at the IRS Standard Rate). Local providers will only receive parking reimbursement.
