Start Date: ASAP - Flexible
Community Based Health Center
Location: Northwest Indiana – approximately 80 minutes from Chicago
BE/BC Family Medicine Physician
Outpatient only – All Ages
Patient Base includes: uninsured as well as Medicaid, Medicare and Insurance.
Hours: Monday – Friday 8am – 4:0pm
No weekends
