Description General Responsibilities: • Provide preventive and primary care for patients in assigned long-term care facilities. • Contribute to effectiveness by identifying short-term and long-range geriatric patient care issues that must be addressed, recommending options and courses of action. • Assess patient health by interviewing patients; performing physical examinations; obtaining, updating, and studying medical histories. • Provide continuity of care by developing and implementing patient management plans. • Collaborate with the nursing staff, patients and patients’ families. Education/Training: Current, unrestricted Nurse Practitioner license in Pennsylvania or Maryland, as applicable; Nurse practitioner national certification as ANP, FNP, or GNP required, or ability to obtain; Ability to obtain DEA licensure/Prescriptive Authority. Experience: Past experience working in a nursing facility or with seniors in other setting.
