Description
- Coverage Needed:
- June 13, 2023 - Ongoing
- Tuesday 7am to Tuesday 7am
- Hospital Call Coverage
- 1:5 Call Rotation
- Clinical Information:
- EMR: EPIC
- Patient Volume 5-10 Per Day
- Procedures: colonoscopies and EGD
- Required:
- Board Certified
- Active IN License
- Must have license in hand at time of submission, or proof that the license application has been submitted
- CV must be in Month / Year format
- Any gaps greater than 30 days will require an explanation on the CV
- Active DEA
- DEA must remain in state to maintain privileges.? Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
- Current Flu Vaccine
- Must be fully vaccinated for Covid 19
- Exemptions only allowed for religious or medical reasons, once approved by the facility.
