The Advocate Medical Group (AMG) is a physician-led and governed medical group committed to delivering the best health outcomes. As part of the Post Acute Network, APNs manage a caseload of skilled nursing home patients in collaboration with Advocate physicians.

Positions are available around the Advocate BroMenn Medical Center, Advocate Christ Medical Center, Advocate Condell Medical Center, Advocate Good Shepherd Hospital, Advocate Illinois Masonic, and Advocate Sherman Hospital.

AMG is seeking an Advanced Practice Nurses (APNs) with one to two years experience preferably working with geriatric patients to join the Post Acute Care Network.
