Momentum Healthcare Staffing is seeking a Nurse Practitioner for a permanent Wound Care position in Troy, New York. This role provides the opportunity to deliver specialized care for patients with chronic or complex wounds, collaborating closely with physicians, nurses, and multidisciplinary teams.

Key Responsibilities:
Assess, diagnose, and manage patients with acute and chronic wounds
Develop and implement individualized wound care plans within NP scope of practice
Perform wound care procedures, including debridement, dressing changes, and advanced therapies
Educate patients and caregivers on wound management, prevention, and self-care
Maintain accurate and thorough documentation of patient care

Qualifications:
Current Nurse Practitioner license in New York (or ability to obtain)
Certification in Wound Care (WCC) preferred
Experience in wound care or related clinical practice
Strong clinical judgment, procedural skills, and communication abilities
