Submission Requirements

BC Advanced Heart Failure Cardiology, Ventricular Assist Device (VAD) Certified - REQUIRED
ACLS - REQUIRED
AHF Fellowship - LVAD experience - REQUIRED
DEA - REQUIRED
Active VA license - REQUIRED
