Description The need is for 2 weeks per month (14 days total) Schedule Hours for Clinic /Hospital 8a-4:30p - onsite inpatient/outpatient, 4:30p-8a - call via phone Schedule Days Clinic /Hospital Hospital/Inpatient Service & Night/Weekend Call Number of patients seen per day in clinic 3-4 patients Call Night /Weekend - Yes What are the days for Call Every day What Time does the Call start 4:30p What Time does Call end 8a Call will be for assigned, unassigned, or both assigned and unassigned patients Both Call response time required?( 15 min? 20 min?) 20 minutes Call Ratio – Call Back % Average Phone consults while on call 3-4/night Average admissions while on call 4-5 patients rounding daily, 1 new consult per day, will see 4-6 outpatients Average patients seen while on call 4-5 patients rounding daily, 1 new consult per day, will see 4-6 outpatients If there are specific dates you need filled please include them here along with the shift times - for example 6/1-5 8a-5p with night call 5p-8a 2 weeks per month (14 days total) Age of patients (Must specify if the provider will see patients under 18) - 18 and under Inpatient /Outpatient - Both Req#22858
