Your daily responsibilities will involve a blend of hands-on physical exams and wellness assessments with essential remote management tasks. On your remote day, you will focus on prescription refills, lab result reviews, and ongoing patient outreach. You will function as a vital member of a multidisciplinary team, ensuring that individualized care plans are followed and that patients receive the education necessary for effective disease management and healthy lifestyle choices.

Schedule: Monday – Friday | 8:00 AM – 4:00 PM (4 Days Onsite, 1 Day Remote)
State License Needed: Pennsylvania NP or PA License

Skills

To be successful in this high-risk geriatric environment, candidates should meet the following criteria:
» Graduation from an accredited Nurse Practitioner or Physician Assistant program.
» A minimum of one year of clinical experience specifically within geriatrics, internal medicine, or family practice.
» Strong clinical intuition and the ability to work independently during in-home or remote care sessions.
» Exceptional communication and active listening skills, with a compassionate approach to treating patients with complex medical and social needs.
» Proficiency in managing electronic health records and coordinating follow-up care efficiently.
