Description ### About the Opportunity\nMedix is seeking an Advanced Practice Provider for a unique role supporting a primary care practice and a nearby skilled nursing facility. This is a consistent 20-hour per week position.\n\nLocation: New Jersey\nFacility Type: Private Practice / SNF\nStart Date: January 26, 2026\nLength of Assignment: Direct Hire\nSchedule: 2 ½ days (20 hrs/week); Tues/Wed 9-5, Thurs 9-1\nState License Needed: New Jersey\nPay: Personalized compensation packages, including competitive rates and applicable stipends, will be discussed in detail with your recruiter.\n\n### Responsibilities\n--> Manage adult-focused primary care (16-20 patients per day).\n--> Provide care to skilled nursing and medically complex patients.\n--> Utilize PointClickCare for documentation in the nursing home setting.\n--> Manage chronic conditions and preventive care visits.\n\n### Required Skills & Qualifications\n--> Minimum 1 year of APP experience.\n--> Active NJ NP or PA License.\n--> Current CDS and DEA registration. * Some information such as the email and phone number are hidden unless you are registered and logged-in
