Description
We are seeking a Physical Therapist (PT) to join Coos County Nursing Home, a county-owned skilled nursing facility and long-term care home serving long-term residents and short-term rehab patients in northern New Hampshire. This is a great opportunity to make a meaningful impact in a rural healthcare setting while building strong relationships with patients and the interdisciplinary care team.Position Highlights:
35 hours per week
County-owned skilled nursing and long-term care facility
Approximately 100 residents
Rural healthcare setting in northern New Hampshire
Opportunity to make a meaningful impact in the community Responsibilities:
Evaluate residents and develop individualized treatment plans
Provide mobility training, therapeutic exercise, and rehabilitation interventions
Document patient care in the EMR and maintain compliance with facility requirements
Work collaboratively with the interdisciplinary care team to support resident outcomes Requirements:
Doctor of Physical Therapy (DPT) or Master’s Degree in Physical Therapy
Active state PT license
BLS certification
Skilled nursing or rehabilitation experience preferred
Experience working in rural or resource-limited healthcare settings is a plus Pay Package:
$50/hour pay rate
$375 travel stipend
