All Medical Schools must be on the California Approved List
All Residencies & Fellowships must be ACGME Approved
No large malpractice issues
Clean NPDB
Three professional references that have worked with the provider within 18 months
Active MI license (IMLC accepted), DEA, and Cs3
Case/patient logs 24 Hematology patients within 12 months
Board Certified or Board Eligible within 5 yearsMyMichigan Medical Center Midland
Hematology/Oncology Outpatient Clinic: Monday-Friday, 8-4:30
Call Required – about 2-5 calls/night
Volume of cases – Approximately 15 visits/day
Outpatient visits consist of new patient referrals, established patients actively in treatment and follow-up appointments
No surgeries or procedures required
BLS required
24 patients with illness related to the practice of Hematology within 12 months
Name Clears to Haley Doede.
