Position Highlights:

• Start Date: January 27, 2025


• Schedule: Monday to Friday, 8:00 AM - 5:00 PM
  
  
  
  • Minimum patient load: 18 patients per day
  
  
  • Mix of pediatric and adult cases
  
  
  
  


• Practice Type: Outpatient only, no inpatient or call responsibilities


• EMR System: Epic

Requirements:

• Active California licensure and DEA prior to the start date


• Certified in Family Medicine (board certification required)


• Experience: Minimum of 3 years in a similar outpatient setting


• PAs: CA Controlled Substance Education Course Certificate


• NPs: Furnishing License with Schedules II authority


• Current BLS certification

Credentialing Details:

• Credentialing timeframe: 30-90 days

  • Positive titers or immunization records for MMR, Hepatitis B, and Varicella Zoster
  
  
  • Tdap immunization (within the last 10 years)
