Description
Our client is currently looking for an experienced Family Nurse Practitioner to fill an Integrated Care opening with a company who is expanding in the Riverside, California area. Interested candidates must have a minimum of 2 years of experience as an FNP and California licensed.
Responsibilities of the Family Nurse Practitioner
- Will support some of our most complex patients, assisting them in navigating a challenging healthcare system.
- Will also provide education and support.
- Nurse Practitioner (NP) will also be the clinical lead/mentor to 2-3 Registered Nurses (RN) Case Managers.
- Round on patients in facilities within assigned location
Minimum Requirements for the Family Nurse Practitioner role
- Active California NP license
- Minimum of 2 years of experience
- DEA Certification
- Preferred experience as a Family Nurse Practitioner (NP), Primary Care Nurse Practitioner (NP), or experience in internal medicine, cardiology, Nephrology or Case Management.
