September: 11–14, 18–21
October: 2–5, 9–12, 23–26, 30–Nov 2
November: 13–16, 20–23, 27–30
December: 4–7, 11–14, 25–28 (Holiday coverage)
Clinical Scope & Volume
The surgeon will provide comprehensive vascular care for an adult population (18+) across two acute care facilities.
Average Callback: 2 hours per day (2 hours gratis).
Facility Size: 150 beds (approx.).
Key Procedures: * Lower extremity bypass.
Amputations.
Emergency vascular trauma/reconstruction.
Inpatient consults.

Provider Requirements
Board Status: BC or BE in Vascular Surgery (Required).
Experience: Minimum of 1 year post-fellowship experience.
Licensure: Active Pennsylvania (PA) Medical License in hand (Required). Willing to license is an option for future dates, but in-hand preferred for May start.
DEA: Required (must have or obtain a PA address for registration).
Documentation: * NPDB self-query dated within 6 months.
Full disclosure of all malpractice history.
7 years of work history or list of last 10 assignments.

Credentialing Information
Timeframe: 90 days standard.
Temporary Privileges: Available only for "true urgent needs" and clean files; must be requested at the beginning of the process.
Critical Deadline: Provider must be fully cleared 2 business days prior to the May 10th start date.
