    • Can take ABIM or ABFM 
      
  • Coverage Needs:
        
    • Need ASAP - Ongoing   
    • 3 month minimum commitment requested   
    • Open to Locums to Perm 
      
  • Shifts: 
        
    •  Monday - Friday 8am-5pm
      
  • Setting: 
        
    • Skilled Nursing Facility  
    • 100% Senior Care   
    • Ages 65+  
    • Medicare Advantage experience is prefered 
      
  • Census:
        
    • 12-18 pts per day 
      
  • Support Staff: 
        
    • Full support staff 
      
  • Procedures: 
        
    • All bread and Butter primary care   
    • Clinical model designed to reduce ER visits and admissions
      
  • Board Certification: 
        
    • Board Certified Required 
      
  • Travel: 
        
    • Facility will cover
      
  • Credentialing timeline:
        
    • Expedited Onboarding hoping for ASAP start 
      
  • Requirements:
        
    • Active CO state license   
    • Active DEA in CO or willing to transfer   
    • BLS
