Compensation/Benefits:

• Salary with Productivity & Performance-Based Incentives
• Sign-on Bonus - $60,000
• Retention Bonus - $20,000 X 4 years
• Comprehensive benefits package including: 4-5 weeks? vacation plus a week of CME and 6% match on retirement after 2 years
• Malpractice coverage of $2M / $6M
