A Home health and telemedicine facility is seeking a Family Medicine Nurse Practitioner for locum tenens coverage.
The facilities are located throughout Pennsylvania - Pittsburgh, Philadelphia, Lancaster, Wilkes Barre, Harrisburg.
This remote position operates Monday through Friday and involves conducting home health assessments for patients with polychronic conditions. Responsibilities include post-hospital stay evaluations and physical wellness visits.
