Position Summary Reporting to the VP of Clinical Services, the Team Manager is responsible for the daily supervision of the clinical team, including, case managers, revisit, per diem, after hours nurse, social workers, spiritual care coordinators and home health aides. Essential Functions Administrative • Evaluates the staffing requirements and recommends changes in the staffing patterns • Prepares the monthly schedule in coordination with the other Team Manager • Reviews and approves the weekly time sheets • Reviews and approves the time off requests in coordination with the other team managers • Maintains schedule of assigned patients to case manager, MSW, SCC and HHA • Recruits, screens and interviews candidates for open positions and participates in orientation of new personnel • Coordinates with the intake/community liaison nurse the assignment of the case managers for new patients • Participates in the establishment of department administrative policies and procedures • Facilitates the interdisciplinary team meetings (IDT) • Prepares the agenda for the monthly nurse’s, MSW SCC and HHA meetings • Coordinates home health aide oversight by case managers • Understands the contracts with providers for delivery of services; such as, pharmacy, durable medical equipment, medical supplies • Ensures compliance with Federal and State regulations and accreditation standards • Monitors the patients’ documentation for compliance with state and federal regulations, accreditation standards, as well as agency policies and procedures • Participates in the development of the department’s annual goals and objectives and quarterly evaluation • Participates in the ongoing evaluation of the clinical component of the computerized patient care system • Participates in Administrator On-Call rotation Clinical • Participates in the establishment of department clinical policies and procedures • Performs annual clinical competency assessment of team’s staff • Provide Clinical coaching to staff through joint field visits • Identifies educational needs of staff and establishes opportunities to address needs • Provides performance coaching to staff, as needed • Provides, in collaboration with other team managers, on a daily basis, clinical oversight of the delivery of care to maintain quality services • Acts as resource to the staff regarding clinical issues that arise, i.e. with providers • Coordinates with the insurance liaison, additional information required by the payer for initial and continues authorization • Participates in the quality management process on a department and agency-wide basis • Participates in the utilization management process including review of new patients & recertification of ongoing patients • Performs service recovery, in response to patient/family complaints and submits appropriate documentation to Quality, Education and Compliance Department (QEC)
Skills
• Current California registered nurse license • Certified Hospice & Palliative Care clinician preferred • MSN/BSN preferred; Associate’s degree with 2-3 years of direct supervisory experience in home health or hospice is acceptable alternate qualification • At least 2 years’ experience working as professional nurse caring for the terminally ill • Prior management experience required, preferably in a hospice/home health care agency • Knowledge and understanding of Medicare, Medi-Cal and private insurance hospice benefits • CPR certification required • California driver’s license and car in good working condition required
