• Board: NP or PA certification required
  
• Licenses: Active PA license required

SUBMISSION REQUIREMENTS

• All required documentation must be submitted no later than 5 business days prior to start
  
• Provider questionnaire required at time of submission
  
• NPDB self-query required at time of presentation, within 6 months
  
• Full disclosure on all malpractice required

Duties & Responsibilities

• Provide clinical support within the Interventional/Electrophysiology department
  
• Manage patient care for a population aged 18-100
  
• Collaborate with the assigned clinical team to ensure 40 hours of coverage per week
  
• Monitor patients postoperatively following interventional and electrophysiology procedures
  
• Maintain accurate documentation within the EPIC EMR system
