Responsibilities
Provide Comprehensive Primary Care:
Offer a full scope of primary care services, including diagnosis, treatment, coordination of care, preventive care, and health maintenance.
Patient Evaluation:
Review patient histories and conduct thorough physical examinations.
Diagnostic Skills:
Evaluate, diagnose, and deliver appropriate treatment and patient care. Make referrals to specialists when necessary.
Clinical Expertise:
Demonstrate quality clinical diagnostic skills, interpreting test results, and recommending suitable management, including preventive health recommendations.
Medication Management:
Prescribe medications and formulate ongoing treatment and disease management plans.
Relationship-Centered Care:
Provide patient-centered care, considering both psychosocial and physical needs.
Record Keeping:
Collect and maintain accurate patient information, medical histories, reports, and examination results.
Team Collaboration:
Communicate effectively with the clinical team to ensure the total delivery of quality care.
Professional Development:
Participate in medical staff educational programs and meetings.
Qualifications
Educational Background:
Doctor of Medicine (MD), Doctor of Osteopathy (DO), or foreign equivalent.
Residency Training:
Completion of a three-year postgraduate medical training from a U.S. accredited residency program in Family Medicine or Internal Medicine.
Experience:
Open to both experienced and newly trained providers.
Board Certification:
Board Eligible/Board Certified in Family or Internal Medicine.
Licensing:
Must possess a current, unrestricted New York State license.
DEA Registration:
Must possess active Drug Enforcement Agency (DEA) registration or be eligible for registration.
Medicare/Medicaid Enrollment:
Must possess active Medicare/Medicaid Enrollment or be eligible to enroll.
Life Support Certification:
BLS or ACLS required.
Communication Skills:
Strong communication skills, both oral and written.
Technology Proficiency:
Comfortable with Electronic Medical Record (EMR) system and Microsoft Office.
