About the Opportunity:

  
• Setting: Article 16 Clinic
    
• Schedule: Monday to Friday
    
• Hours: 9am to 5pm or 8am to 4pm
    
• Patient Population: Adults (Ages 18+)
    
• EMR: PrecisionCare
  

  
• Review MD’s referral and persons receiving services medical record prior to determining speech/language therapy services
    
• Evaluate and assess the persons receiving services’ needs and level of functioning in the areas of: speech, language, cognitive & social aspects of communication, voice, augmentative and/or alternative communication (ACC), and eating /management of diet (dysphagia)
    
• Devise treatment plans based on the assessment; administer treatment involving the application of communication and/or feeding aid modalities
    
• Consult with supervisor when appropriate to implement recommendations
    
• Provide training to persons receiving services, families, direct care and/or outside agencies, if warranted
    
• Communicate effectively with discharge planning from rehabilitative programs
    
• Participate and attend care plan/team meetings for all persons receiving Speech-Language Therapy services
    
• Ensure all documentation is completed through EMR within the time frame established
    
• Perform other duties, as needed
  

  
• Bachelor's Degree
    
• NYS Speech Language Pathologist (SLP) license
    
• TSSLD
    
• Solid assessment and documentation skills
  

  
• 1+ year of Clinical experience
