Join an established neighborhood primary care/geriatric clinic, using a value based care model, to ensure patients are seen & heard year round preventing complications from multiple chronic conditions. ROLE
• Manage other Physicians while maintaining a small patient panel of 100
• Monday thru Friday, 7:30a- 5p; NO WEEKENDS + Protected Admin Time
• Average 3-5 Physicians per clinic
• Local & Regional Travel
Care team: Scribe, MA, Social Worker; Onsite Lab & Pharmacy PACKAGE
Salary $300k
Quality Bonus annually

Skills
• Ability to obtain/maintain medical license in state of Illinois
• Minimum of 1 Year Experience; Leadership experience preferred
• Board Certified/Board Eligible: Internal Medicine, Family Med. or Geriatrics
