Requirements:
- ABOS Board Certified or TRULY Board Eligible in Orthopedic Sports Medicine (must become certified within 5 years) – [REQUIRED] * no exceptions
- Valid IL & IA license or Active IMLC Willing to Obtain Licenses - [REQUIRED]
- Strong arthroscopic and open sports surgery skills [REQUIRED], shoulder, knee, hip [PREFERRED]
- Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the field of practice - [REQUIRED]
- DEA - [REQUIRED]
- BLS Certification and maintain recertification - [REQUIRED]
- Clean malpractice and license history – [HIGHLY PREFERRED] / [PREFERRED]
- Does the provider require visa support – No J-1 or H-1B (Note in Additional Submission Details) – [REQUIRED]
- Submit Self-Query NPDB pulled within 30 days from submission at time of presentation - [REQUIRED]
