Requirements:
- Must be Board certified in Orthopedic Trauma Surgery or a Truly board eligible - [REQUIRED]
- Must have fellowship in Orthopedic Traumatology or Adult Reconstruction/Total Joint Arthroplasty - [REQUIRED]
- Have an active license in both Iowa and Illinois - [REQUIRED]
- Strong clinical and surgical expertise with a commitment to comprehensive hand and upper extremity care - [REQUIRED]
- Open to both graduating fellows and experienced orthopedic surgeons
- Does the provider require visa support – H-1B cap exempt (Note in Additional Submission Details) – [REQUIRED]
- Submit Self-Query NPDB pulled within 30 days from submission at time of presentation- [REQUIRED]
