- Monday-Friday - 8a-5p with 1 hour unpaid lunch
- Setting: Clinic No Trauma/Not Rated
- Required Procedures/job Duties: Managing Chronic Conditions: overseeing ailments like dementia, arthritis, diabetes, and cardiovascular disease. Preventative care & assessments: Conducting comprehensive physician exams and cognitive screenings. Medication Management: Safely prescribing and adjusting medications to avoid polypharmacy complications. Care coordination: Managing transitions between hospitals, rehabilitation centers, and home care.
