Join a Community Health Center with 100+ Providers (30+ Physicians and 75+ APP’s) in 20+ locations as they grow.

They are expanding their Post Acute Care / Long Term Care Facilities/Skilled Nursing Facilities teams.

Primary responsibilities include overseeing patient care at area nursing homes in post-acute care, long term care and assisted living.
•  Facilities are established, long term and short term
•  100 – 280 beds depending on location
•  Cover ONE facility
•  ~18 patients / day
•  NP's on staff and NP's do discharges
•  PM&R & Behavioral Health physicians on site.
•  Monday - Friday
o  no nights
o  no weekends
o  8 AM – 5 PM
•  Dragon dictation with AI scribe
•  EMR: eClinicalWorks
•  Part-time positions available

Benefits:
•  Base salary - 320k - 350k DOE
•  Sign on - 20k
•  Relocation reimburse up to 20k
