Description We are seeking a Physical Therapist (PT) for a full-time contract opportunity for a home care position in Houston, TX. The physical therapist is responsible for evaluating/assessing needs of referred patients and formulating treatment plans in a home care environment. Responsibilities included but not limited to are: -Provides therapy services defined in treatment plans -Meets the patient's goals and needs and provides quality care by assessing and interpreting evaluations and test results; determining physical therapy treatment plans in consultation with physicians or by prescription. -Work cooperatively with physicians, case managers, and adjustors -Evaluates effects of physical therapy treatments and fit of prosthetic and orthotic devices by observing, noting, and evaluating patient's progress; recommending adjustments and modifications. * Some information such as the email and phone number are hidden unless you are registered and logged-in
